TRICARE and Surgery Abroad: What's Covered and What's Not

TRICARE Is Not the Same as VA Healthcare

First, the distinction that trips up many veterans: TRICARE and VA healthcare are separate systems. VA healthcare serves enrolled veterans. TRICARE serves active-duty service members, military retirees (20+ years), Guard/Reserve members, and their families. If you separated before 20 years and aren't activated Guard/Reserve, you likely don't have TRICARE — your coverage is through VA enrollment, employer insurance, or the ACA marketplace.

The Elective Surgery Question

Here's the practical reality: TRICARE will reimburse for medically necessary care obtained overseas, including from non-network providers. However, elective procedures (cosmetic surgery, LASIK, hair transplant) are not covered by TRICARE regardless of location — the same exclusions that apply in the US apply abroad.

For procedures that are medically necessary but you're choosing to have them abroad (joint replacement, dental work, bariatric surgery), TRICARE may cover its portion if the procedure would have been covered in the US. Pre-authorization is strongly recommended before traveling.

The 25% cost share on a $12,000 procedure in Colombia means you'd pay $3,000 out of pocket — potentially less than the copay on a $50,000 procedure in the US.

How to File an Overseas Claim

  1. Get itemized bills in English (or with translation) showing procedure codes, dates, and provider credentials
  2. Complete DD Form 2642 (Patient's Request for Medical Payment)
  3. Attach proof of payment (credit card statement, wire transfer receipt)
  4. If you have other health insurance, file with them first — TRICARE is second payer
  5. Submit to International SOS / TRICARE Overseas Program
  6. Reimbursement timeline: typically 4–8 weeks after clean claim submission
