Where Canada Private Wins
Simplicity. Everything happens within the system you know. No flights, no passport, no foreign hospitals, no language considerations. Your Canadian surgeon, your follow-up care, your physiotherapy — all local.
Continuity. Your surgical team and your follow-up team are in the same country, often the same city. If a complication arises at week 6, you go back to the same surgeon who operated on you.
Employer coverage. Some employer health spending accounts (HSAs) or private insurance plans cover surgery at accredited Canadian private facilities but not international procedures.
Low-cost procedures. For procedures under $10,000 CAD (hernia repair, minor orthopaedic, some eye surgery), the cost gap with Colombia narrows once you factor in flights and time. Canadian private may be simpler for less expensive procedures.
Where Colombia Wins
Cost. This is the decisive factor for most patients. When knee replacement is $30,000+ CAD in Canada and $12,000 USD all-in from Colombia, the math speaks for itself. For dental work, the gap is even wider — a single implant in Canada costs more than a multi-implant package in Colombia.
Availability. Private surgical facilities in Canada are limited, particularly for cardiac surgery, neurosurgery, and bariatric surgery. Colombia has a deeper bench of internationally accredited hospitals performing these procedures for international patients year-round.
Recovery infrastructure. Colombia, particularly Medellín, has purpose-built medical tourism infrastructure: recovery houses with nursing staff, bilingual patient coordinators, established aftercare networks. Canadian private surgery sends you home — which is fine if you have support there, but less ideal for patients living alone.
Tax benefits. Ironically, the CRA's METC may provide a larger effective tax credit on Colombia surgery (where you pay the full amount out-of-pocket) than on Canadian private surgery (where partial employer coverage reduces the eligible amount).
Decision Framework
Choose Canadian private if: the procedure costs under $10,000 CAD, you have employer coverage for private surgery, you strongly prefer recovering at home, or your health makes international travel inadvisable.
Choose Colombia if: the procedure costs over $15,000 CAD privately in Canada, you're comfortable traveling, you want to maximize savings on a limited budget, or the procedure you need has limited Canadian private availability (bariatric, cardiac, complex spine).
